Submitting Business

  • Important Information:
    • All applications must be submitted within seven days of the application date. The effective date will be the requested date on the application, WHICH must be later than the application date contingent on underwriting approval.
      • One-Time Application fee of $25.
      • If any question 1 through 6 in Part III is answered “YES,” the applicant is not eligible and the application should not be submitted.
    • Do NOT mail any new applications that are faxed through the fax application program. This will create a double entry.
    • $1 per month added to premium for Fraternal Membership Dues.
    • A telephone interview (used randomly on all new business).

    Required Forms:

    • Replacement Form (required with any application replacing a Medicare Supplement or Medicare Advantage plan)
    • Creditable Coverage Determination (Guarantee Issue)—If yes, proof of coverage must be submitted with the application.
    • Bank Draft Form—Bank Draft Authorization and voided check must be included.

      • All faxed applications must be accompanied by an “Assured Life Assn/Woodmen of the World” fax application transmittal cover sheet.” Please submit only one application per transmittal form.
      • It is important to keep all faxed paperwork until the policy has been received.
      • Do not collect any money on applications that you intend to fax in for processing.
        • The initial draft will occur on the billing cycle immediately following the issue date.
        • First renewal premium draft will occur on the Policy due date.
      • You will be contacted if there is a problem with your transmission.
