How to Express Applications with Equitable Life & Casualty Insurance Company:

Agents can now begin the Express Application process online at www.equiline.com. One of Equitable Life and Casualty Insurance Company’s tele-underwriters will call an agent’s client to complete the remainder of the application. Agents do not need to be present for that call. Agents will be notified via email once the policy is issued.

If you would not like to start the Express Application process online, please complete the steps below.

  1. Using the medical questions from the application, pre-qualify the applicant. If the applicant does not qualify for the Level product but does qualify for the Graded product, be sure the prospect will accept that coverage before proceeding.
  2. Have the applicant read the Notice of Information Practice, Privacy Policy, and Life Buyer’s Guide. The Privacy Policy must be read by the applicant or the agent must read it to the applicant prior to calling to start the Express application. If the applicant states they have not received a copy of the Privacy Policy, or have not had it read to them by the agent, the interview cannot be completed.
  3. With your client present, call 1-866-551-1781 to complete the express application. You must call to have the Medical questions verified by an interviewer. Interviewers can be reached Monday through Friday from 7 a.m. to 7 p.m. MT and Fridays from 7 a.m. to 6 p.m.
    • Before calling, please be sure your client has the following information readily available:
      • If this policy will replace an existing life or annuity policy (if “yes”, be sure the applicant has signed the replacement form)
      • Bank name, routing number and account number; draft date (between the 3rd and 28th of the month)
      • Name and address for beneficiaries (Be sure the applicant knows if there are contingent beneficiaries)
    • If the applicant is not the policy owner, the policy owner’s authorization and agreement are required during the telephone interview.
    • The telephone interviewer will ask you, the agent, for the following information:
      • If this an Express or Regular application (your answer should be “Express”)
      • The telephone number you are calling from
      • Your name and agent number
      • If there is a splitting agent involved—if yes, you will need to provide the splitting agent’s name and agent number and the split percentage.
      • Client’s first, middle, and last name
      • Product information:
        • Face amount
        • Plan applying for (Level or Graded)
        • Will the automatic premium loan be included?

DO NOT COACH THE APPLICANT! It is recommended that you explain what will take place but you must not participate in the interview. Our interviewers are trained to identify coaching and, if identified, the interview will be terminated.

Once the interview is complete, send the Replacement form (if required) to the Home Office. The Replacement form must be received within 14 days of the application date or commission will be charged back. Submit the application via mail, fax it to 888-352-5126, or upload the application on www.equiline.com.

Note: Please remember that Equitable Life & Casualty Insurance Company does NOT accept a credit card number or a debit card number. The policy must be paid by bank draft so be sure your clients know if they are paying via their checking or savings account.

DO NOT COLLECT PREMIUM OR A VOIDED CHECK. A request for funds is electronically submitted to the customer’s bank upon approval of the policy, and Equitable receives payment electronically. Premiums will be deducted automatically from your clients’ bank accounts for the first AND subsequent months.

How to Submit Regular Applications with Equitable Life & Casualty Insurance Company:

  1. Using the medical questions from the application, pre-qualify the applicant. If the applicant does not qualify for the Level product, be sure the prospect will accept the Graded product before proceeding.
  2. If the applicant is applying for the Level benefit, answer all the medical questions in the application including height, weight and cigarette usage. If the applicant is applying for the Graded benefit, check the box indicating the applicant has chosen the guarantee issue product.
  3. Complete the replacement form if replacing any other Life or Annuity policy (if required in your state).
  4. Have the applicant review the Notice of Information Practices, Privacy Policy, and Life Buyer’s Guide.
  5. Do not call to complete a point of sale interview. An interview will only be completed if the Rx or MIB check reveal information not admitted on the application.
  6. Complete application in its entirety. Areas commonly left unanswered that will delay the underwriting/issue process include the following:
    • Relationship of owner and/or beneficiary
    • Face Amount
    • Automatic Policy Loan
    • Height/weight
    • Name and address of company whose existing coverage is being replaced. (This is vital as many states require the replacing company to notify the replaced company within a specific time period.)
    • Missing forms
    • Missing signatures
  7. Submit the application by mailing the application, faxing to 888-352-5126 or uploading it on www.equiline.com.
