Complete the Phoenix annuity application, disclosures, and any state required forms.
The required annuity product application is determined by the state in which the owner signs the application,
The producer is required to be licensed in the “owner signed at” state only.

Common not-in-good-order (NIGO) issues:

Missing signatures
Capacity of signature must be identified when applicable
A physical street address is required (P.O. Box is not acceptable as the only mailing address)
Incomplete beneficiary information
Missing date of birth
Incomplete replacement questions
Missing proper forms
Missing data such as:
- Line of Business – IRA, Roth, 1035 Exchange
- Benefit Selection/Rider Option (as applicable)
- Asset Allocation
Submitting IMO and contact not clear/Missing cover sheet
Producer contracting – delays related to due diligence

Suitability Completion

The Phoenix Suitability Form must be completed and submitted for all application submissions. Other company suitability forms will not be accepted.
If the form is not completed, the annuity will not be issued. If the producer does not recommend the sale, the annuity will not be issued.
If the source of the premium is a mortgage, reverse mortgage, or a home equity line of credit, the annuity will not be issued

Initial Premium and Temporary Insurance Agreement OR Void Check and authorization to draft

Commission Payments: Carrier pays twice weekly.Commissions close Tuesdays and Fridays. Payments from Tuesday close and receives EFT on Friday and Friday closes and receives EFT on Wednesday; carrier pays by check or EFT.Advance Payments: Carrier pays upon receipt of first payment and delivery receipt returned IGO. They pay twice weekly. Commission closes are Tuesdays and Fridays. Tuesday closes receives EFT on Friday and Friday closes receives EFT on Wednesday; we pay by check or EFT.
