Scott & White Health Plan and MetLife now have a dedicated telephone number for SeniorCare and Vital Traditions plan members. Members may now call 855-676-9337 to speak to representatives designated specifically for assisting members enrolled in into the dental plan.

Submitting Business

Scott & White requires a Scope of Appointment (SOA) to be transmitted with enrollments. Agents must use their branded SOA form. Scott & White will not accept a generic, CMS-approved SOA or another carrier-branded SOA.

Applications must be faxed. Scott & White does not accept applications via overnight mail.

How to Submit Business with Scott & White Health Plan:

Scott & White applications can be faxed to 1-254-2698-3567.

  • Scott & White does not require a transmittal or special cover sheet.
  • Agents will receive a daily log of the applications received and will be asked to confirm that the list is accurate. If the list is inaccurate, or if an application is submitted but does not appear on the daily list, the agent should notify Ritter immediately.

Commissions

Commissions are paid on a monthly basis.

•Custom Benefit Plans can also order supplies from Scott & White on your behalf.

• The website will verify that you have completed all required certifications.

• There is a maximum number of enrollment kids that you can order; however, Custom Benefit Plans can contact Scott & White to approve a higher amount of kits.
