How to Submit Business with Aetna Health and Life:

Paper Applications

  • Mail new applications paying by check to:

Required Forms:

  • Completed application
    • Make sure to complete Section 4 Health Questions (unless Open Enrollment or Guaranteed Issue)
    • Do not submit application with “Yes” answer in Section 2
  • Completed HIPAA authorization
  • Initial premium (one mode) and the Policy fee (For faxed applications, the initial premium will be drafted)
  • Replacement Form
  • Documentation of disenrollment, termination of coverage, or change in service area notification from carrier or group plan (if the applicant qualified for Guarantee Issue)
  • Bank Service Plan documentation (if the client wants automatic bank withdrawal)
